Dr. Jones: My 7 yr old son has been on Daytrana now for 5 days and he has had excessive talking. In fact he has gone into the restroom and continued to talk to himself in there. Does this fade over time? He always talked a large amount but this is compulsive. Also he has had bad pains in his stomach, will this decrease over time?

HI Dr. Jones, I have many issues, but to try to narrow it to my main problem for some ideas from you I'll try to focus on what thoughts yu might have. I had a previous eating disorder w/ bulimia at one time due to slow motility thru out my whole gi tract, and had a complete removal of colon 6/06,from my colon being perferated from a colonoscopy. I'm very underweight around 80lbs. at 5'2 3/4 in.but do wk. full time ,have a son in college and daughter with adhd in h.s. I do what needs to be done but suffer from extreme anxiety, loss of motivation or enjoyment no sex drive ( my poor husband) you name the meds, I've tried them. Currently on klonopin, ambien, percocet estrogen patch zelnorm prevacid feel very discouraged. Any advise you can perhaps suggest to me to pass on to my dr. would greatly be appreciated. Sincerely, Shari Oh by the way I'm 54 yrs. old

Dr. Jones,
My question is basic: Changing from Adderall to Daytrauna patches.
This child had finally found relief in some form with Adderall 90mg. The child states they are more focused and report cards give evidence to success in school.
With starting Daytrauna 10mg qd : is it comparative to Adderall 90 mg effectiveness.
I would assume you would need a higher dosage. I was giving instructions to to increase daily. First, day 10mg, Second day 15mg, Third day 20mg to a maximum 30mg is this typical in prescribing for 45 Kg. Is there harm in starting at a higher dose so the let down from Adderall would not be so significant?

forward to your answer. Thanks.

Sharon, Thank you for the great question. In general, all stimulants can cause some reduced heat tolerance due to their norepinephrine enhancing effect. Adderall is potentially more of a problem than Daytrana because Adderall has the strongest Norepinephrine effect of all the stimulants. Depending on the time of day, Adderall XR might be less of a problem since it reaches maximum effect in 6-7 hours and then the effect begins to decrease. Daytrana, if left on 9 hours will continue to increase norepinephrine levels gradually until taken off, and if left on more than 9 hours it may not reach its maximum level for another 10 to 12 hours.

Adding a low dose of Tenex (also known as the generic guanfacine) would reduce the risk of heat intolerance and improve focus at the same time. Ask your doctor about going back to Daytrana and take with 1/2 to 1mg of Tenex in the morning for best results, but even if taken in the evening it would add some protection.

Dr. Jones,

Thanks for the great info on Daytrana. My 10 year old son (50 lbs) was on Daytrana 10 mg for about 2 weeks. This was his first ADHD medication. It was working well, but I got nervous after he got sick while playing soccer. I became concerned that the heat (90 degree day) and increase in body temp maybe caused too much medication to be released into his system. So, we switched him to Adderrall XR about 3 weeks ago. He's been at 10 mg for about 2 weeks. But he says he felt more focused on Daytrana. He also has very little appetite on Adderall XR, has more rebound in the evenings and it lasts only about 9 hours. We are thinking about trying Daytrana again. We would take it off before he goes to soccer, but he has approx 20-30 minutes of recess and phys. ed outside during the school day. We live in Florida, so it gets to 90 degrees for a good part of the year. I was wondering if you could offer any insight about potential danger posed by the patch releasing too much medication, and if this is a concern during short durations (30 minututes) of activity during outdoor play in hot weather, particularly after the patch reaches doubles its release of medication after 4-5 weeks as you explained. Thanks!
